Parent/Guardian Consent & Liability Waiver

I am the parent or legal guardian of the child or children named in this registration, and I have the authority to give consent on their behalf.

If I am registering a child who is not my own, I confirm I have their parent or guardian's permission to enter them. I understand their parent or guardian must also complete a consent form before the child can race.

I give permission for my child to take part in the ORA Fit Fest Kids Race, organised by Mase Limited trading as ORA Fitness Studio (ORA Fitness Studio). I understand the event involves physical activity including running, functional fitness stations, and other forms of physical exertion, scaled to suit young athletes.

I confirm that, to the best of my knowledge, my child is in good health and has no medical condition or impairment that may prevent or limit their safe participation. I have told ORA Fitness Studio about any medical conditions, allergies or other needs that event staff should know about.

I understand that participation carries inherent risks, including but not limited to muscle strains, sprains, falls, broken bones, heat-related illness, and other injuries. I accept these risks on behalf of my child.

I agree that my child will follow the instructions of ORA Fitness Studio staff, marshals and volunteers during the event. I understand that my child may be withdrawn from the race if staff believe it is unsafe for them to continue.

I understand that I am responsible for supervising my child at the venue when they are not racing, and for collecting them at the end of their race.

In the event of an injury or medical emergency, I authorise ORA Fitness Studio staff to arrange first aid and/or emergency medical treatment for my child if I cannot be reached immediately.

To the fullest extent permitted by law, I release ORA Fitness Studio, Mase Limited, and their employees, volunteers, contractors and agents (the released parties) from all claims, demands and causes of action arising from or in connection with my child's participation in the event.

If I am racing as my child's duo partner, I also accept these terms for my own participation.

Media Release
I understand that photos and videos will be taken during the ORA Fit Fest Kids Race for promotional purposes. I grant Mase Limited trading as ORA Fitness Studio the right to use images, video or audio of my child for marketing, social media and other promotional materials in print and digital formats.

If you do not consent to the use of your child's image or video for marketing purposes, please email management@orafitnessstudio.co.nz with your child's name before the event.
